I don't think it's your Squid. I have seen the same
(?)'ERROR_CONNECT_FAIL' error repeatedly reported under Squid 1.1.9 for a
similar client; in this case, a W95/Netscape system running on a
poor-grade ppp dial-up connection through another ISP.
