new documentation

Hi all

There is slightly new documentation out - only a couple of sections difference.

All sections excluding "proxy accelerator mode" up till
"Additional Software and misc" have been done (I have done the hierarchy
section sort of and re-written the acl's section. Hopefully the ACL re-write
will save people some pain...)
