1.2b23-4: Support for ip-filter

I have made a preleminary patch that adds direct IP-Filter support to
Squid 1.2beta. I have not been able to test this patch myself as I do
not have a machine where ipfilter compiles, but it should work..

I would preciate if someone using IP-Filter and who knows about Host:
headers and IP-addresses could test this patch and report to me if it
works or not.

The patch is available from http://hem.passagen.se/hno/squid/ as usual.
