You're being overloaded at times, it would appear (service times are
quite long, particularly on hits).
You aren't telling us what we really need to know to be able to point
you in the right direction:
What OS and version?
What hardware? You're running a very highly loaded Squid--you need big
hardware.
What is your current Squid configuration? (I see you're using diskd,
but beyond that we can't see much from what you've given us.)
