Many people misread this part of the FAQ.

cache_mem only ADDS to the amount of memory Squid is using, it does not
limit the memory usage.

Quid rule of thumb:

Memory usage = 10 MB per GB of cache + cache_mem + 20MB.

Then add some margin on top of that.

I think you are actually referring to the printout telling how much data
Squid has cached on disk, printed shortly after the cache index has been
reconstructed..
