Then your PAM configuration for the service "squid" is not correct.
See your copy of the PAM system administrators guide, or look at how
the other PAM services are configured on your system.
PAM configuration is usually found in /etc/pam.conf or
/etc/pam.d/<servicename>.
In most cases just duplicating the configuration of ftp/pop3/telnet
works just fine.
