I don't see anything wrong with your squid.conf. It does look like you
are using an older release of Squid; I would recommend you upgrade to
the most current 2.5 STABLE release. It's possible there is a bug in
the older code that is causing this.
In your original email you remarked that you weren't being prompted
for proxy authentication. According to the setup you've provided, the
only reason you would get prompted is if your restrict-im acl wasn't
matching properly. You may want to take a look at that. Remember that
when you are using a _regex acl, if you want . to mean a literal
period, you need to use \. because . in a regex will match any
character.
