[squid-users] Multiple authentication, access denied

I use squid-2.5STABLE4 to log http traffic. The problem is that sometimes
my users can't authenticate themself at the beginning of browsing. They
have to repeat the whole authentication procedure two times, three times
etc. Sometimes they are allowed to connect to an external server (the
proxy is behind a firewall) and sometimes they get an "access denied..."
message. In addition I can't see any related messages in the logfiles.
I haven't mentioned I am new to Squid.

Does somebody know what the solution is?
