Lets see: the rest of the world see this:

www.mywebsite.com has address 64.34.199.43
what does it look like to your test machine?
and also to the squid server?
Your logs show that you are running an rfc1918 DMZ (192.168.0.5 etc)
you need to check that the domain name is returning the 192.168.x.x
number of the internal domain to your internal machines.
