This means the request was denied.

> 1176708775.804 3 192.168.1.10 TCP_MISS/403 1507 POST
> http://www.example.com/test/testing/welcome.asp - DIRECT/123.123.123.123
> text/html

And this request was allowed.

Check your http_access rules, and cache.log.
