[squid-users] OWA accelerator authentication weirdness

Running 3.0 my accelerator for OWA works fine.
The browser user authentication prompt is "A user name and password are
being requested by ..."

The same configuration on 2.6STABLE6 or 3.1.0.3 -
The browser authentication prompt is now different: "Enter user name and
password for ...".
When I enter attempt to log in with new dialog, the dialog is cleared
and redisplayed with the following in the log:

If I select CANCEL in the dialog, it changes back to " A user name and
password are being requested by ..." and I can log in normally:

etc.

Any ideas on what might be different on 3.0 than the other two?

My config is basically as follows:
https_port ip:443 cert=/usr/share/ssl/owa-gbateam/owa-gbateam.pem
defaultsite=abc.com
cache_peer 172.16.4.64 parent 443 0 no-digest no-query originserver
login=PASS ssl sslflags=DONT_VERIFY_PEER
sslcert=/usr/share/ssl/exchange/exch-owa.pem name=owa-server
