This behaviour shows up when there is a crash during the shutdown of the
worker process. The master only sees the child ending on a crash and
restarts.

You will need to do some tracking down to find out what the crash is
about. Hopefully it is mentioned in cache.log. Some tricks with testing
on a production machines are detailed at
http://wiki.squid-cache.org/SquidFaq/BugReporting

When you know what it is please check bugzilla, there are a few known
causes of shutdown crashing. If you find its a new one, please add a new
bug entry about it.
