Does everything work if you remove cache_dir lines from squid.conf?

If not, try OS configuration hints posted at
http://wiki.squid-cache.org/Features/SmpScale#Troubleshooting

The last one is especially relevant to FreeBSD even though you may not
see the "message too long" error unless you enable higher level debugging.
