Added the following note:

"You can _optionally_ also set a visible_hostname parameter. Usually
this is not necessary; Squid by default will use the machine hostname.
If you have a pool of Squid servers, you may need to set
visible_hostname to prevent forwarding loop errors. In our case, we
chose to set visible_hostname because the machine hostname included our
client's business domain name, and we didn't want that appearing in the
Via http headers sent to external web servers. (This is a special case
for this particular client; this shouldn't be a concern for most
environments.)"

Is that reasonable?
